Starting with the evaluation, not the prescription

Anxiety and depression are the two things people most often arrive already labeled with, sometimes accurately and sometimes not. Before we decide what to do about it, I want to understand what we are actually looking at.

That means the 60-minute evaluation covers more than a symptom list. When did this start, and what was going on then? Is it constant or does it move? What does your sleep look like? What is happening at work, at home, in the relationships that matter? Have you been through a loss you have not had room to process? Are you carrying something you have never said out loud?

Some of what gets called depression is depression. Some of it is grief, exhaustion, a situation that would flatten anyone, or a life that has quietly stopped fitting. Those need different things, and sorting it out is the point of the hour.

Medication as one component

When medication is the right call, it can genuinely change what is possible. It can lift the weight enough that you can think again, or make therapy feel workable when it previously felt like too much. That relief is not a lesser kind of help, and dismissing it is not strength.

What medication cannot do is process grief, teach boundaries, or change a situation that is genuinely making you unwell. So we will be specific about what we are asking it to do, and we will check whether it is doing that.

Therapy, where it fits

I encourage psychotherapy for most people dealing with anxiety or depression, and I will say so plainly if I think it is the piece that would move things most. I do not provide therapy myself — this practice is medication management — but I would rather you have both than have me quietly substitute one for the other.

Function, not just mood

The question I care about is not only whether you feel better. It is whether you can do the things that matter to you. Get up. Work. Be present with people. Sleep and have it count. Symptom scores can improve while a life stays stuck, and that is not success.

Sleep, stress, and the rest of the picture

Sleep, nutrition, hormones, alcohol, stress load, and how much of your week belongs to you all affect how you feel, and all of them interact with medication. Looking at them is not an alternative to evidence-based treatment. It is part of an honest assessment of what is driving how you feel, and occasionally it turns out to be the more useful lever.

In-person appointments are at 9631 N Nevada St, Suite 209 in Spokane. Telehealth is available to adults anywhere in Washington. Both use the same 60-minute evaluation and 25-minute follow-up format.

Insurance

Think Beyond Psych is in-network with most major Washington plans, including Premera Blue Cross, Regence BlueShield, Asuris Northwest Health, First Choice Health, First Health Network, Kaiser, Aetna, Cigna, United Healthcare, and URM. Jesse verifies your specific benefits before the first appointment so you know what to expect rather than finding out afterward. This list is not complete, so if you don’t see your plan, ask anyway and Jesse will check.

We are not able to see patients covered by Medicaid, Apple Health, or other state-funded programs — including as self-pay. This is a federal patient protection rule, not a preference. If that is your coverage, Jesse will gladly help point you toward providers who accept it.
