Room to say the true thing
A lot of people in the postpartum period have already been asked how they are doing, several times, by people who needed the answer to be fine. So they said fine.
The reason the initial appointment is 60 minutes is that some things take a while to get to. Intrusive thoughts that frightened you. Anxiety that has nothing to do with anything rational and will not turn off. Feeling detached from a baby you love. Rage that arrives out of nowhere. Not recognizing yourself. None of that is disqualifying, and none of it is something I need you to soften before you say it.
What this practice provides
Psychiatric medication management for adults, including postpartum anxiety and depression. That means evaluating what is going on, deciding together whether medication has a role, and following it carefully rather than starting something and disappearing.
To be clear about the boundary: this is a psychiatric medication practice. I am not your obstetric provider, I do not provide psychotherapy, and I do not manage the medical side of recovery from delivery. Where those need attention, we coordinate rather than pretend.
If you are breastfeeding
Tell me at the first appointment. Whether you are nursing, planning to, or have stopped, it is directly relevant to which options are on the table and how we weigh them. It is a conversation, not a rule about what you are allowed to do.
Anxiety after a baby is often the bigger story
Postpartum depression is the phrase everyone knows. Anxiety is at least as common in what I see, and it gets missed more often, partly because vigilance about a newborn looks reasonable from the outside. If your mind will not stop running worst cases, if you cannot sleep even when the baby is sleeping, if you are checking things you know you already checked, that is worth an appointment.
What else is going on matters
Sleep deprivation, a difficult birth, feeding that is not going the way you were told it would, no help at home, going back to work too soon, and grief you did not expect all belong in the conversation. So does what your relationship looks like right now. None of this is separate from how you feel.

The Spokane office is at 9631 N Nevada St, Suite 209. Telehealth is available to adults anywhere in Washington, which for this particular stretch of life is often the difference between getting seen and not. You do not have to arrange childcare to be seen by video.
Insurance
Think Beyond Psych is in-network with most major Washington plans, including Premera Blue Cross, Regence BlueShield, Asuris Northwest Health, First Choice Health, First Health Network, Kaiser, Aetna, Cigna, United Healthcare, and URM. Jesse verifies your specific benefits before the first appointment so you know what to expect rather than finding out afterward. This list is not complete, so if you don’t see your plan, ask anyway and Jesse will check.
We are not able to see patients covered by Medicaid, Apple Health, or other state-funded programs — including as self-pay. This is a federal patient protection rule, not a preference. If that is your coverage, Jesse will gladly help point you toward providers who accept it.

If you are having thoughts of harming yourself or your baby, or you are in crisis, call or text 988 or go to your nearest emergency room now. Do not wait for an appointment. I do not treat active crises, active psychosis, or active substance use, and I do not prescribe benzodiazepines.
You do not have to be at a breaking point to make the call.
